New Caregiver Program

The Healthy Choice Program is a voluntary premium discount program from the Employee Health Plan (EHP) that rewards you for taking action to manage your health. The program provides resources to help you save on your premium and support your overall well-being. As a *new caregiver at Cleveland Clinic, you will be eligible to join the New Caregiver Program. When you participate and complete the required tasks, you'll earn a discount on your current year's health plan premium, which will be reflected in 1-2 pay cycles.

The New Caregiver Program is available to new caregivers* at Cleveland Clinic hired between Jan. 1-Oct. 31 and have elected the health plan. 

Getting Started: Create Your Healthy Choice Account

Your Healthy Choice portal is a valuable tool to help you stay on track and meet your goals. From the portal, you can view your Incentive Program, Program Requirements, activity tracking, premium levels and much more. 

Once your account is created, view the Program Requirements in your Healthy Choice portal and complete all required tasks no later than Dec. 28.

  • If you have a spouse on the plan, they will need to create their own account to participate.

*2026 New Caregiver Eligibility Definition: 

A caregiver is eligible for the New Caregiver Program if they meet one of the following criteria:

Caregiver has not been employed at Cleveland Clinic within 12-months prior to their most recent hire date.

OR

Caregiver is newly enrolled in EHP benefits due to Open Enrollment or a Life Event change. If you're newly married, please contact an EHP Wellness Specialist at 216.986.1050, option 3. 


Under HIPAA, EHP like other health insurers, is permitted to access health data for the purposes of claims payment, health program development and treatment coverage. As with any of our healthcare plans and programs, plan member privacy is protected in full compliance with HIPAA.
