Accessing Service Prior to Receiving Health Plan ID Card

If you need services during this time frame and prior to your benefit elections being processed, you may be asked to pay upfront for services. This can be adjusted at a later date when the Third Party Administrator (TPA) processes your benefits selections. If you need an appointment urgently and have selected your benefits but do not have your medical plan ID card yet, you may ask the Cleveland Clinic scheduler to select “EHP Pend” as your payer when scheduling the appointment. Your provider office can also call EHP to confirm coverage.  Please note, if you need services from an in-network non Cleveland Clinic provider prior to receiving your medical plan ID card, you will have to pay upfront for services and then complete  the applicable reimbursement form below.

If you have to pay out of pocket for prescriptions prior to being activated with CVS/Caremark, you can complete a form and file for a reimbursement.  Remember, stay with Cleveland Clinic pharmacies, Cleveland Clinic Home Delivery or CVS/Caremark to maximize your prescription drug benefit.

Viewing your Health Plan ID Card

ID cards can be viewed and/or printed prior to their arrival in the mail after your health plan coverage selections have been processed. Once registered, you will have access to your ID card and claims information. In addition, you will have access to your plan-specific provider network.

To view your medical plan ID card, log into Aetna Health using your Aetna Health credentials or download the Aetna Health app.

Viewing your Prescription Drug Plan ID Card through CVS/Caremark

To view your CVS/Caremark prescription plan ID card, visit the CVS/Caremark website.

Once registered and logged in, click on Plan & Benefits on the navigation bar and then Print Member ID Card.  Within this area, you can also request a new card.
