Review 2025 benefit changes:
• $75 co-pay will be added on outpatient surgeries - ambulatory surgery centers, hospital outpatient and hospital locations
• $35 co-pay will be applied to specialty virtual visits to align with in-office specialty visits
• Chiropractic visit limit will change from 30 to 10 visits per benefit year
• The specialty of Neurology will be removed from the Aetna supplemented specialties in Florida for the EHP plan
• Services for removal of impacted wisdom teeth will be covered under your dental benefit, administered by Cigna Dental, and not under the medical benefit
• The EHP Plus plan infertility network requirements are changing for 2025. There will no longer be a requirement to use the Cleveland Clinic Women’s Health Institute when members live within 130 miles of Cleveland Clinic. EHP Plus members seeking treatment for infertility must use an Aetna Infertility Institute of Excellence (IOE) provider. If there is no infertility provider/facility in the IOE within 130 miles of where you live, seek treatment with an Aetna network infertility specialist. Precertification will still be required through Aetna’s Fertility Advocate.
• Effective Jan. 1, 2025, the Fisher-Titus provider group which serves the Norwalk, Huron County and central Ohio region are in-network for EHP and EHP Plus plans
Struggling to Pay Your EHP Emergency Department Copay?
$250 can be difficult to pay all at once. Did you know that you can sign up for payroll deduction when your copay is greater than $50. You can pay emergency visit copays, with payroll deduction and remit over 10 pay periods ($25 per pay). Speak to your registrar during your visit about setting up payroll deduction.
You must sign up for payroll deductions during the registration process. To enroll in payroll deduction, you will need to have both your badge number and employee number. Don’t carry your badge with you all the time? Take a picture of the back of your badge, where the numbers are visible. Carry it with you when you need to reference it in the ED and take advantage of payroll deduction.
